Why is Tuvok reverred to as Lieutenant all the time in the first season (I'm rewatching Voyager from the start), while his pips clearly indicate the rank of a lieutenant commander and in TNG a lieutenant commander (Data, Geordi) is mostly referred to as commander?

Yea.. it was a major wardrobe mistake during season 1. Tuvok was promoted to the rank of LCDR in "Revulsion" (year 4.)

Such rank mistakes are common in Trek.

I think VOY may have had the most on screen rank changes of any other show. I may be wrong about that.

Yea.... I think that's probably because the lack of real military service by someone maintaining the uniforms for the cast.

Miles O'Brien was also another example - they couldn't get his rank right....

Actually, that's because they didn't pin down that he was a Chief Petty Officer until later on, so he was an Ensign in 'Encounter at Farpoint' and 'All Good Things..." then a Lieutenant other times, then they finally just scrapped it and called him a Chief Petty Officer. I don't believe they gave him an insignia until Deep Space Nine, however.

Anyway, Tuvok was a Lieutenant in the first season of Voyager.
