in "where no man has gone before" The tombstone gary mitchell created read "James R. Kirk" when we all know that his middle name is Tiberius.

It's not necessarily a mistake. Kirk could've legally changed his middle name.

Because this came before any mention of "T" as his middle initial, this would establish his middle initial as "R" and the later references to the "T" would contradict what was first established.

Listen carefully afer McCoy sees the large rabbit & little blonde girl in Shore Leave. He clearly says "it's like something out of 'Addless' in Wonderland". Not a big deal really ... but something that could easily been remedied with another take. De Kelley did get a little tongue twisted at times. Some of those fun, funky little errors make the series all that more endearing as the years pass by.

that was the first episode with Kirk.Kirks miffle name wasnt offiated untill the moviee "The undicovered countery".
so no, at the time, we didnt all know his middle name was Tiberius or anything with a T.

In The Changeling, after Uhura's memory was wiped of everything including language, she got frustrated with basic English & started ranting in fluent Swahili.

Sam was his middle name, only im called him by that name.
and they never said he only had 1 soon, they just only showed 1

the defintion of the word "TREK" was expanded long ago to include
a long and often difficult journey

Her memory was completely wiped of all knowledge including language. Not to completely rule out the ridiculously remote possibility that someone else on the ship would have known it and would have been able to teach it to her, but it would be completely ridiculous to start off teaching her a language that would be of absolutely no use to her ability to return to duty to the point that she could rant fluently in frustration over her ability to pick up the most basic words of a language necessary to do her job.
